Why bother with a questionnaire?
Most disappointing Secret Santa gifts aren't the result of anyone being careless. They happen because someone was handed a name, knew nothing about that person, and had to guess. A questionnaire replaces the guess with a few facts, and it does it without turning the exchange into a shopping list.
The most valuable question on the form isn't the one about what you want — it's the one about what to avoid. Allergies, scents, dietary requirements and the thing you already own six of prevent far more bad gifts than any wishlist. Answer that box even if you skip the rest.
If you're organising, send the questionnaire out at the same time as the draw link and give people a deadline a week before shopping starts. Answers that arrive after everyone has bought their gift are just a list of things nobody got.

- Should I say exactly what I want?
- One or two specifics help a lot, but leave room for your Santa to choose. The single most useful box is the one about what to avoid — allergies, scents and dietary needs save far more bad gifts than a wishlist does.
- Is it rude to say what you don't want?
- The opposite. Nobody enjoys buying something that gets quietly rehomed in January, and being told 'no more mugs' is a gift to your Santa, not an insult.
- Can I print it instead?
